Frequently Asked Questions

What should I bring to my first visit?

You will need to bring a picture ID and your insurance card to assist with completion of registration forms.

Can you write a prescription or give a referral before I see a physician for the first time?

We believe it is not good medical practice for us to prescribe any medicine to a nonpatient. Furthermore, it is important for our doctors to have a full evaluation of your medical history and current medications prior to prescribing or providing referrals.

What if I need to talk to a doctor after the office is closed?

After regular business hours, call the health center’s regular office phone number. Our answering service will page the on call physician to handle your call. After hours, the on call physician does not have access to your chart, so for matters that are not urgent (completion of forms, test results, scheduling an appointment, referrals, medication refills), please call during our regular business hours.

Does your office participate in HMO or PHO health plans?

Yes, we accept the following HMO plans and participate with the following PHOs:

We also participate with the following PPOs/POS:
